Using the Learning Health Network (LHN) framework, we are:
accelerating and scaling outcomes improvement by developing community partnerships,
sharing and implementing evidence-based guidelines, and
supporting teams in ongoing quality improvement initiatives.
What is an LHN?
LHNs are collaborative systems that encourage rapid learning by bringing together families, health providers, researchers, and community organizations. LHNs use data to inform quality improvement initiatives and achieve better outcomes.
